2008-51 Asbestos Floor Tile Removal Johnson Middle School Quote
is due on or before Wednesday , June 11, 2008 at 10:00 AM
This work is
expected to commence June 30, 2008 and be completed in a week's
time. The scope of the work consists of: 1. Remove asbestos floor
tile from perimeter first floor hallway, approximately 5,300 square
feet. 2. Obtain all required permits. 3. Proper disposal of all
debris.
All work to
be performed under strict compliance with all State and Federal
Official Agency and Local regulations.
The responsible
vendor who submits the lowest price will be expected to pay prevailing
wages and supply the Town of Walpole proof of insurance coverage
including Worker's Compensation. The successful vendor also shall
be required to submit documentation that all employees to be employed
in the work subject to this quote have successfully completed a
course in construction safety approved by United States OSHA that
is at least 10 hours in duration. The selected contractor must furnish
a 50% payment bond.

The scope of the work consists of: 1. Supply and install approximately
5400 sq. ft. Armstrong Standard VCT 2. Supply and install Skim Pro
Floor patch skim coat. 3. Supply and install 4" vinyl cove
base. 4. Supply and install transition strips as needed.
