Worker's compensation
Employees who have a work related injury must notify their supervisor immediately (within 24 hours). Supervisor and employee must complete the appropriate forms. The forms must be forwarded to the Human Resource department at the Town Hall within 48 hours of the injury.
Supervisor Report of Accident - Intake Form (see 2nd page for clarification of Cause; Body Part; Nature; Occupation)
Medical Release Authorization - employee's signature required
Norfolk County Retirement (NCR)- Notice of Injury - only employees who have deductions taken for NCR
