Business Certificates

Filing for a Business Certificate: Commonly known as a "d/b/a" or "doing business as" form.

A Business certificate is a local registration of a business that is located within the Town and is required by MGL Chapter 110, s.5Filing a Business Certificate FAQ Information

A Business Certificate is not a license to do business in the town.  Its purpose is primarily for consumer protection and public information purposes.  Additional licenses/permits issued by other Town Departments or State Agencies may be required in order to do business.  Please check with the Building Inspector/Zoning Enforcement Officer (508-660-7324) to determine if the business is an allowed use in the intended zoning district.

Business Certificate Forms

Business Certificate Application Form is only required for a business when first applying for a business certificate.  Multiple departments are required to sign off on the application before processing it at the Clerk’s Office.

Business Certificate -  Notarized Form is used when all owners are unable to sign the certificate in the presences of the Town Clerks Office in which case all owners must sign in the presence of a notary.   Please enclose a self-addressed, stamped envelope, check or money order payable to the Town of Walpole along with the notarized form.  A registered copy of the Business Certificate will be returned to you.

The fee for a Business Certificate is $40.00 and the Certificate has a 4 year expiration date. 

Business Certificate Amendment Form is used to process one of the following changes:

  • Discontinue or Retire a Business
  • Deceased or withdrawal of one of the owners
  • Change of Owner Residence
  • Change of Business location
  • MINOR change in Name of Business

The fee for each amendment is $25.00
